Financial Policy
Bella Faccia Dental Spa, Center for Comprehensive Cosmetic and Implant Dentistry is committed to providing you with the best possible care. In order to achieve this goal, we need your assistance in understanding our payment policy.

Charges for your consultation and treatment will be determined and explained to you at the time of your appointment. Payment for services are due at the time the services are provided.

If you have insurance, please present proof of coverage at the time of your initial visit. Some insurance coverage information may be gathered at the time your appointment is made. We are eager to help you receive your maximum allowable benefits. We will be happy to provide assistance in estimating your coverage, deductibles and co-payments and file your insurance claims. Please keep in mind that insurance is a benefit to help defray the cost of dentistry. Patients are fully responsible for payment of all fees associated with the dental treatment. The estimated patient portion is due at the time treatment is rendered. (Please see separate form for insurance patients.)

All emergency dental services must be paid at the time services are provided.

Cancellation Policy
In order to ensure that we can provide the finest in dental treatment, we must utilize our resources efficiently and effectively. For your appointment, we allot a specific amount of the doctor and hygienist’s time just for you. As a courtesy, we ask that you give us notice at least 48 hours in advance for appointment changes or cancellations.Cancellation of appointments with less than 48 hours notice create a tremendous burden on our practice in terms of loss of opportunity for other patients to be seen and in terms of overhead. We thank you in advance for the courtesy of providing 48 hours notice for appointment changes or cancellations.

We realize that unforeseen circumstances will occur, but require a minimum of 48 hours notice for cancellation or changes to appointments. An Appointment Cancellation Fee will be charged for cancellations or changes in appointment with less than 48 hours of notice.
